- Are the company Director and Secretary details correct?
- Have you enclosed a 288a form for any appointments of company Directors or Secretaries?
- Have you indicated the issued capital, and is it correct?
- If you have indicated that a full list of members is enclosed, have you enclosed it?
- Has the form been signed?
